Description
The harm caused to patients during healthcare or as a result of it, which prolongs hospitalization, causes disability at discharge, or both, and is not due to the patient's underlying condition, defined as an adverse event, is the cornerstone of patient safety programs. Adverse events have been identified as a global issue for patient safety, leading to the implementation of numerous actions. According to the World Health Organization, 1 in every 10 patients experiences some form of harm during their healthcare, and every year, over 3 million people die as a result of unsafe healthcare. In low- and middle-income countries, this figure can reach up to 4 in
every 100 people, with more than half of the cases involving preventable adverse events. The frequency of adverse events varies depending on numerous factors, which requires each country, region, and healthcare institution to make an additional effort through actions that allow for a proper characterization of their scenario from a patient safety perspective.

Objective
To determine the incidence of tracer adverse events identified in the adult population treated at Méderi between January 2021 and december 2024.
Key findings
The study aims to determine the incidence of tracer adverse events and the characteristics of the adult population treated at Méderi from 2021 to 2023 as a basis for continuous improvement actions and processes at the institution.
